Boise YMCA Swim Team 2010-2011 Registration
for Continuing and Returning Members
(Anyone who is currently swimming for or has swam for the Boise Y Swim Team
at any time between Sept. 2009 and today's date.)
All families currently swimming with our team must return a completed 2011 Registration Packet for Continuing and Returning Members if you intend for your swimmer to continue with the team beyond December 31, 2010. If you joined the team within the past few months and have already completed the 2011 paperwork, you do not have to fill it out again! The packet for continuing and returning members will be available in October.
Complete all parts of the 2011 USA Swimming Registration Form, Team Membership Requirement Form, Health History Form, Swimmer Code of Conduct, Parent Pledge, and Parent Service Responsibility Form. Mail all forms to Patty Stratton, 2017 S. Roosevelt St., Boise, ID 83705. Do not give forms to the YMCA or to the coaching staff!
Swimmers whose paperwork is not received by December 15th will not be allowed to practice with the team or compete at a meet as a Boise Y Swim Team member until the completed paperwork is received by Patty.
